How does tumescent anesthesia help recovery?

The advantage of tumescent liposuction is the liposuction recovery time.  Tumescent means ‘firm’ and consists of swelling the fat layer with a saline solution containing lidocaine.  By expanding the fat layer with fluid, we give ourselves space to work.  The fluid also helps to remove the fat which is solid.   The tumescent anesthesia also separates the nerves and blood vessels away from the fat so they do not become injured during the procedure.    As a result, the tumescent anesthesia is much gentler on your body and speeds liposuction recovery.

The use of local anesthesia has many other  advantages over general anesthesia including:

1.  Its is much safer as general anesthesia is riskier than the liposuction procedure itself.

2.  The tumescent anesthesia only numbs the skin and fat layer.  Everything deeper like muscle, nerves, blood vessels and organs still have sensation.  As a result we cannot accidentally touch these vital structures without you stopping the procedure!

3.   After the procedure you will be right back on your feet immediately and go home numb.  The anesthesia will most like wear off that evening.  There is no long recovery nor bedrest needed and this avoids common postoperative complications like blood clots, etc.

How much time is usually enough for liposuction recovery?

In general, most patients liposuction recovery is rather quick.  You will be back on your feet immediately after the procedure and able to resume most normal activities.   The anesthesia will wear off gradually that evening and supplemental pain medication may be necessary.  Many patients get by with Tylenol alone but you will be provided with stronger pain medications.  The soreness subsides after several days and it is rare for a patient to ask for more pain medication.  You will be wearing a compression garment for several weeks after the procedure to deal with swelling.  For about 2 days postoperatively, you will be draining from the small incisions.  This drainage actually speeds your liposuction recovery as absorbing the anesthesia and melted fat would prolong your recovery.  So besides some soreness and swelling to limit you, it is safe to return to normal activities rather quickly.  Since their are no big incisions or stitches, there is no fear of hurting anything with too much exertion.  A long weekend of 3-4 days is the time off most patients take.  Of course everybody is different and other factors like number of areas, pain tolerance, your occupation, etc. should be taken into account when deciding on your liposuction recovery time.

Does liposuction recovery depend on the body area treated?

Liposuction recovery varies depending on how extensive the procedure (multiple sites, amount of fat, volume removed), patient pain tolerance and health, and surgical technique. In general, you will feel more than 50% recovered at one week and 80 to 90% recovered at two weeks in a moderate volume (1 to 3 liter) liposuction. Inner and  outer thighs tend to hurt more than abdomen and arms.  Back, flank and bra rolls tend be sore a little longer.  Liposuction recovery from chin is much quicker.

When can I expect my Liposuction Recovery to be complete?

The time after liposuction is a time for your body to heal and for your results to become evident.  In the areas we removed fat from, your body will fill the empty space with fluid which causes swelling.  You will required to wear a compression garment to keep this space closed and squeeze the fluid from the area.  We will also teach you to do massage to help reduce the swelling.  Physical activity will also aid your liposuction recovery.   Your body will begin to reorganize the fat that is left in the treated area.  The skin tightening from the laser liposuction will begin several weeks after the procedure and continue for months.   So it is about at the 3-4 week time period where we will know where you will settle down.  However, final liposuction recovery can take 3-6 months.

What is Laser Liposuction?

Liposuction in any form involves removing fat from the body.  It used to be done only in the operating room under general anesthesia.  It involved using very large suction cannulas and results were variable.  In the 80s, liposuction under local anesthesia was developed.  It is now the preferred method  of liposuction using very small microcannulas.  It gives much better results with little to no downtime.   Around 2006, Smartlipo Laser Liposuction was invented.  This involved the insertion of a small laser fiber through the same incisions.  The heat generated by the laser caused skin tightening, melted fat and cauterized blood vessels.  We now have many different types of laser liposuction devices available.

So how is Laser Liposuction used?

Liposuction with a cannula is very effective to remove fat from anywhere on the body.  However it is not just about removing fat.  It is the condition of the fat layer that we leave that is more important.  This is very difficult to achieve, especially in areas with small amounts of fat.   Being too aggressive can leave unsightly grooves and dents.  Plus removing fat doesn’t do anything for the stretched out skin.  This is where laser liposuction comes in.  The heat generated by the laser liposuction helps to melt fat cells.  This fat can then be suctioned or allowed to drain out.  So the laser is perfect to melt fat in smaller areas and smooth out the fat in larger areas.   The heat generated by the laser also stimulates some immediate skin tightening and induces new collagen formation.   As a result, the overlying skin contracts to the new, smaller fat layer giving the perfect liposuction results we want!

How do I choose a laser liposuction procedure?

Smartlipo laser liposuction by Cynosure was the original laser on the market.  There are many competitors now like Slim Lipo, Cool Lipo, Pro Lipo, etc.  The only differences between these lasers involves the wavelength of light used and power.  Smartlipo MPX uses 1064 and 1320 wavelengths that have research to back their clinical effectiveness.  However, there are no studies to show the advantage of one laser over another.  Remember the effects of laser liposuction are due to heat delivery.  All of these lasers deliver heat to the fat and  skin.  It is the consensus among the liposuction community, that the laser is unimportant compared to the amount of heat delivered.  So it is the surgeon who is performing the procedure and his experience with laser liposuction that will give you the best results.

So what should I look for when choosing Laser Liposuction?

The main things to require for your  liposuction procedure include tumescent anesthesia, microcannula liposuction and laser liposuction.  These three components will give you the lowest risk of complications, the best results and fastest recovery.  Ask your surgeon specifically about each of these techniques and make sure he will be utilizing them.  Skipping any of them can effect your results and risk complications.

Is Laser Liposuction safe?

The effectiveness of the laser depends on heat delivery.  With the use of heat there is the risk of burns.  Many of the lasers now have temperature controls but this doesn’t replace the experience of the surgeon to prevent such complications.  Check your surgeon’s reviews, pictures, etc.  However, the incidence of burns from laser liposuction is very low and rare.

How fast will I see results from laser liposuction?

The heat generated from laser liposuction will cause some immediate skin tightening.  However with the swelling and loss of fat it will be hard to see for several days.   As the swelling subsides and skin tightens, you will begin seeing the smooth, smaller fat layer and tighter skin.  This is the fun part as you see more and more results everyday!  It usually takes 3-4 weeks to begin to see where you will settle down and 3-6 months for final results.

Thank you for choosing Beleza Medspa and Cosmetic Surgeon Dr. Lawrence Broder for your SmartLipo laser liposuction procedure!  At this point you are probably experiencing a range of emotions: from excitement to fear.  Rest assured these feelings are very normal and we would like to provide you with smartlipo liposuction austinsome further information about the SmartLipo procedure so that you may proceed with confidence.

This is an elective cosmetic procedure and it is important for patients to expect realistic results. Dr. Broder will speak to patients directly to ensure that they understand the nature of this procedure. It is crucial that your expectations are in line with what the procedure can deliver.  SmartLipo laser liposuction can provide targeted fat removal and skin tightening to the areas we treat. Dr. Broder guarantees at least a 50% improvement, and typically far surpasses this goal.  SmartLipo is not a weight loss procedure, nor a guarantee against further weight gain.  You will always have the results we provide if you maintain your weight or lose weight.  We do leave a layer of fat and these fat cells can still enlarge.  So it is possible to out eat your liposuction!  We hope this procedure will be a jump start to a new and improved program of diet and exercise for our patients. Dr. Broder will start by taking a medical history and perform a physical exam to determine if you are an appropriate candidate, and do not have any medical contraindications. It is not unusual that we detect an umbilical hernia that must be repaired prior to the procedure.  We also frequently run into medications that must be discontinued prior to the procedure.  Many anti-depressants interfere with the metabolism of the local-anesthetic lidocaine and must be temporarily stopped. Anti-inflammatories such as Aspirin, Motrin, Advil, etc., must be discontinued since they can make you bleed.  We may be required to draw some blood for screening labs and sometimes require an EKG or other type of medical clearance.  Dr. Broder will ensure utmost patient safety and will not perform the procedure unless completely convinced that you can safely tolerate it.

On the day of the procedure, you will be asked to take a mild sedative that will help relax you before coming in.  We will also provide sedatives, when necessary, during the procedure to make you as comfortable as possible.  All medications will be given orally. Using this regimen, the procedure is well tolerated by the majority of our patients.

The first step will be to complete your consent forms and ask any questions you have.  Once you change, Dr. Broder will take pictures and place markings on the body areas we will be treating.

We will then prepare your skin with an antiseptic. Dr. Broder will inject a few areas with tiny amounts of lidocaine.  He will then use a circular punch to make small ‘adits’, so that we may introduce instruments into the fat layer.  We work in the layer of fat between the skin and the muscle. Dr. Broder will then use a small cannula to introduce a saline solution containing: lidocaine (numbness) and epinephrine (no bleeding) into the fat layer. This solution lifts the fat layer off the muscle and makes it firm, hence the term ‘tumescent anesthesia’.  All underlying structures: muscles, organs, major veins, etc., still retain sensation. If we were ever to get too close to them, you would feel it and stop us. This is why ‘awake’ liposuction is so much safer. We cannot hurt you unless we try to!

The administration of the anesthetic is the only part of the procedure where you will feel any discomfort. There may be a little pain or irritation before the lidocaine fully numbs the area. The discomfort is fleeting and the entire anesthesia process is quick.  We will then give you 20-30 minutes to allow full anesthesia to take effect.  Once we start you will feel some vague movements but there will be no pain!  We always keep a reserve of anesthesia in case we feel we need to top up the dose, to ensure maximum patient comfort.

The amount of lidocaine is calculated based on your body weight. This dictates how much anesthetic fluid we have to use and how many areas we can do in one session. This is one of the limiting factors, and why we are only able to treat a few areas at once. It is always safer to return another day to do more areas. This is for your safety and we never deviate from this rule.

Once we are certain you have good anesthesia, we will begin the procedure. Your skin will be cleaned again. Dr. Broder usually begins with microcannula liposuction. This means he will use very fine liposuction instruments to suction fat from the area. The smaller cannulas allow finer work and less risk of irregularities.  As this is as much an artistic procedure, as it is a scientific one, Dr. Broder will work carefully to shape and sculpt your body.  This takes talent, experience and a careful eye.  Dr. Broder is guided by the feel of the fat layer as well as how it looks. Good results are not entirely dependent on how much fat he removes, but how much he leaves and what condition he leaves it in.   His goal is to leave a smooth, uniform and well contoured fat layer that will make the area look thinner, tighter, and more shapely.

Dr. Broder usually introduces the SmartLipo laser towards the end of the procedure. The laser generates heat in the fat layer and helps to melt fat cells. Dr. Broder uses this to smooth out the fat layer.  The heat also causes skin contraction and collagen stimulation. Dr. Broder will apply the heat from the laser in the areas that need skin tightening. Dr. Broder will alternate between suctioning and lasering until he has the best cosmetic result he can produce.

At the end of the procedure you will be placed in a compression garment. This is a tight, medical-grade under garment that must be worn at least 3-4 weeks after the procedure, sometimes longer. As you can imagine, when we remove fat cells from your fat layer, an empty space is left. Your body naturally doesn’t like empty spaces and will fill it with lymphatic fluid that will manifest itself as swelling. Your body will eventually repair the area that we treated and this swelling will resolve over the next few weeks. Some residual swelling may remain for up to 3 months after surgery. This is when your part of the procedure begins. The swelling responds to 3 things: compression, massage and exercise.  You must wear your compression garment as much as possible until we clear you to stop.  We are trying to keep that empty space closed, so that it heals tighter and smaller.  We will also teach you to massage the area and push the fluid away.  The lymphatic fluid causing the swelling is normally propelled through your body by muscle action, so exercise will help you further.

The small adits we made will be left open, so that you can drain until they close in 48 hours. You will drain the remaining anesthetic fluid and any melted fat. This is done to shorten your recovery time.  We ask that you keep all your initial dressings on for the first 24 hours after surgery, essentially up until you take your first shower.  No baths, hot tubs, or swimming are permitted for one week.

You will go home feeling numb and this will last for several hours.  Unlike the prolonged recovery expected from general anesthesia, your recovery time after the local tumescent anesthesia is minimal. We do require that you do not drive and have a ride to and from the office. We will give you some oral pain medication to take as needed.  We find that the majority of our patients do not require a lot of pain medication. The tumescent anesthesia you were given is protective of your tissues and allows a relatively pain free recovery.

You will be required to return to the office the next day so that we can assess the status of your dressings and answer any questions you may have. You will be asked to return in one week for your massage, and weekly after that for follow-up visits. We are always happy to see you at any time if you have any concerns. Since there are no large incisions or sutures, you will not have any activity limitations or restrictions.

Your immediate results will be variable. As we previously discussed, swelling will inevitably settle in. Rest assured, the fat is not returning! Just give it time, compression, massage and activity and those great results will return!

Complications from this procedure are extremely rare. Your physical and screening prior to the procedure were the first step in preventing complications. We will keep you on a prophylactic antibiotic to prevent infection. Some patients will experience bruising. We encourage these patients to use Arnica Montana, a natural supplement that helps with bruising and swelling. If you are an easy bruiser, we will prescribe a vitamin K supplement when necessary. Other more serious complications can be avoided as long as we stick to your lidocaine limits, and you follow our post-operative instructions. Of course, we are a fully equipped medical office and can handle any unforeseen problems.

The final results of SmartLipo laser liposuction may take 3-6 months to be fully seen. Your body will go through a healing process that will remove the swelling, reorganize the remaining fat, and tighten the skin. We will take pictures at 3-4 weeks out and provide you with before and after photos so that you may see your transformation at that point.

Dr. Broder: Hello, I’m Dr. Lawrence Broder, MD, medical director of Beleza Medical Spa in Austin, Texas. I’m here today to talk to you briefly about the HCG Medical Weight Loss Diet. I guess if you’re watching this video, you have some weight to lose. You’re wondering how this diet will help you lose that weight. We all know there are millions of diets out there. Most of those diets, whether they’re food supplements or medications, will help you lose some weight initially. The problem is that there is no long-term weight loss. We want you to lose weight and keep it off. I see patients every day asking me, “Doctor, I work out. I go to the gym every day. I eat right. I’m not losing weight.” The problem is, is to understand the dynamics of weight loss and weight gain. You have to understand, one pound of fat equals 3500 calories. Let’s say you have 20 extra pounds on your body. That 20 pounds equals 70,000 calories. That means you have to be negative 70,000 calories to lose that 20 pounds. Not only do you have to eat well, you have to be negative in caloric intake to somehow to lose that weight. We can say, “Maybe I can exercise that off, maybe I can burn those calories off my body.” That’s another problem. The problem with exercise, that if the average person walks a mile, they burn about 100 calories. If you want to burn off a pound of fat, you have to walk 35 miles. I bet that at the end of that 35-mile walk you’re going to be hungry and you’ll probably eat more than 3500 calories. Plus the appetite stimulation continues, and you tend to compensate for your exercise. Exercise is not a answer to initial weight gain. It is extremely important for your health and your longevity. It is unbelievably important for your maintenance of weight loss. In initial weight loss, it has a role along with diet. Diet is probably more than 80 to 85 percent of initial weight loss. Exercise during that initial weight loss by watching your diet will make you lose more weight than if you didn’t exercise alone. What are we basically looking at here? The thing we have to really feel and look at in ourselves is that we are addicted to food. We are addicted to food for many reasons. The food industry, due to government subsidies, has made food so cheap that we spend a smaller percentage of our income on food now than our parents did. We eat out more. We see more commercials. We are given foods that are less nutritious and we eat larger portions. It just takes 100 calories a day over a month to gain one pound a month. You can imagine over the years, how much fat you can gain. That’s why we’re here talking about this right now. Unless we break this food addiction, you can lose weight initially but you won’t keep it off. This is where the hCG diet Austin comes in. The hCG diet Austin was proposed in the 1950s by a doctor in Italy. It had a resurgence in the ‘70s and now it is popular again. It is very controversial in the medical literature. There are studies that support it. There are studies that don’t support it. The diet is coupled with a hormone. That is the hGC, human chorionic gonadotropin. It is made by the placenta of pregnant women. The theory is that in pregnant women, the hCG along with maybe other substances released by the placenta, allows the fetus to access the fat stores of the mother. Even if the mother is not taking in enough calories, the fetus can access her fat stores to keep energy coming in to the fetus to continue growth. That’s how this diet works. The other part of the diet, you have to remember with hCG is the very low calorie diet, which is 500 calories a day. To give you some perspective, you probably need 2000 to 2500 calories, depending on your activity level and your sex, just to meet your basic daily core of needs. This is 500 calories. It’s almost a starvation diet. Then you’d ask, “Why don’t I just go on a 500 calorie a day starvation diet and I’ll lose weight?” You will lose weight, but you’ll lose it in ways you don’t want to. The body is designed through evolution to save its fat stores until we have food again. You will breakdown your protein, your muscle. Your blood sugar will drop and you will feel terrible and you will not be able to function. This is where the hCG comes in. The hCG appears to allow you to access your fat stores and to keep your blood sugar levels up so you can function and you feel good. It also allows you to access the fat in the areas around your body, specifically the abdomen and the thighs, and lose inches. You feel good, you lose weight and you lose inches with the hCG diet. hCG is FDA approved for ovulation induction in infertile women. It is not an FDA-approved indication. This is an off-label use. The good thing is that we use a dose that is one fiftieth of that dose. I have searched far and wide and have not found any side effects or any reason that you have to worry about the hCG. The dangerous part of the hCG program is the diet. To do a 500-calorie diet on your own is very dangerous. We do a physical that I supervise. We draw blood work and we follow you. You need this because it is very dangerous to be on a 500-calorie diet by yourself. With proper physician monitoring, it is very safe. The diet is very strict. Look at it as a ‘nutritional bootcamp’!  It teaches you to eat very small portions. It teaches you the good things to eat, the protein, the vegetables, the fruits. You can lose one to two pounds a day sometimes. The good thing is at the end of the diet you have learned how to eat and you have learned to break the food addiction. This is like going cold turkey from smoking, except that the hCG is a crutch that helps you go cold turkey. Then at the end of the diet, you have learned that you can eat less and survive and lose weight and feel good. That’s the important part. We are not looking for you to put the weight on again to go back to your normal. We want you to learn that you can eat those smaller portions and eat a healthier diet and lose weight too, and feel good. We use an topical cream form of hCG. There is also the injections. That’s how it was first used. The topical form is just as safe and effective, and avoids daily injections of medications. It is probably the diet that really breaks your food addiction and teaches you how to keep it off. The hCG, along with the diet, helps you get through it. It is a four to six-week program. We include various things in the diet like a digital scale, books, videos to teach you how to eat better. We have a nutritionist. We have other counseling. We want you to succeed. We want you to lose weight. We want you to have the ability at the end of the diet to have more muscle mass, to have less fat and to have the tools to continue on the rest of your life. What are the Risks and Side Effects of Coolsculpting?

Coolsculpting is a very safe procedures.  It was FDA-approved for fat reduction in September of 2010 and has been safely used in Europe for approximately 5 years.  The reason why it is so safe is that the cold exposure is not to such an extreme that it immediately kills fat cells;  rather, the cold exposure initiates natural programmed fat cell destruction, which is why it takes 2 months to see the results.  The exposed fat cells naturally degrade and the byproducts excreted by your body.  The only contraindications to undergoing Coolsculpting is if you have rare conditions of cold-intolerance.

Coolsculpting may be slightly uncomfortable during the first few minutes of treatment, but it is more of a strange suction and cold feeling than pain and gradually turns into a barely noticeable numbness.  This strange feeling is greatest with the largest treatment head/applicator, and less with the small and medium applicators.  Immediately after Coolsculpting, the treated area may have mild to moderate tingling, numbness, and soreness.  Mild bruising is sometimes seen, but is uncommon.  The soreness may last for 2-3 days and feels like the muscle soreness you get after a round of intense exercise.  The tingling and numbness feels just like the sensation your skin has after intense cold exposure.  It lessens over time and is usually limited to 2-3 weeks.  Coolsculpting is not known to cause long-term nerve damage.
